Enrollment & Forms

Upon enrollment you will be given multiple forms to fill out.

There are two forms that you will need to print off, the first is the Child Information Record and the second is the Medication Permission and Instructions.

Please have all forms completed and returned on the first day your child comes to the child day care.

Once your child is enrolled we ask that you download the Remind app in order to receive important news and announcements such as day care closures due to inclement weather.
